Dr. Marvin Diaz, DC, MSACN
Founder of Eleva Performance & Rehab, is a Montgomery County native and bilingual English- and Spanish-speaking chiropractor. He earned his Doctor of Chiropractic and master’s degree in Applied Clinical Nutrition with honors from Northeast College of Health Sciences.With advanced training in sports injury care and dry needling, he combines chiropractic care and active rehabilitation to support people with sports injuries, auto accident injuries, and spine-related concerns. A first-generation Honduran American, Dr. Diaz is committed to personalized care that helps his community return to the activities that matter most.

What to expect
After booking, look for an email with a link to your new-patient forms. Please complete them online before your visit.

What to bring
Please bring a photo ID, relevant imaging, test results or recent medical records, and a list of medications you take. For an auto accident visit, bring any available claim information and details about the accident.

Eleva is currently a self-pay practice. We’re awaiting a response from Blue Cross Blue Shield and will share insurance updates as soon as they are confirmed.
